Appointments It is extremely important that your appointments are kept and that you are seen regularly so that we are able to complete your orthodontic treatment in a timely basis. Let us remind you that we require lengthly appointments to be made early in the day, while we reserve late afternoon appointments for short checks in order to accomodate more patients at a more demanding time of day. If you are unable to keep an appointment, kindly give us a 24 hour notice. If you miss three appointments without notice, there is a nominal fee for any broken appointments thereafter. Appliances If a band, bond, archwire or appliance becomes loose or broken, please call us as soon as possible. It is in the best interest of your care to have the problem corrected as soon as possible. Excessive breakage due to abuse will necessitate a replacement charge. Toothbrushes, headgear and removable appliances are to be brought to every appointment. If for any reason the appliances are not being worn as directed, please advise us. Loss or destruction of any of these appliances will necessitate a charge. If your treatment requires the wear of elastics, it is most important that they are worn at all times, even while eating. They may be removed only for tooth brushing or while changing them. Failure to cooperate in this respect will automatically insure that treatment can never be properly completed. If you should happen to run out of elastics, just give us a call and we will be happy to mail more to you. For removable orthodontic appliances, insurance is available through our office for loss or breakage. Tooth Brushing and Oral Hygiene Teeth should be brushed and flossed after every meal. Snacking is strongly discouraged. Brushing should be continued for as long as possible to get the teeth thoroughly clean. Failure to do so will encourage decay and decalcification of the teeth. Vigorous rinsing of the mouth with water after meals is highly recommended when brushing and flossing are not practical or possible. Fluoride mouth rinses are highly recommended. Additional information can be provided upon request. Cooperation Full cooperation by the patient and parents is essential to successful treatment. Failure to cooperate will result in prompt notification of the patient, parent or guardian. The treatment fee is estimated assuming full patient cooperation. If the treatment should extend beyond the normal time limit due to lack of cooperation, an additional monthly fee will be charged. The treatment fee does not include lost, broken or damaged appliances or excessive broken appointments. Regular Dental Visits It is important that regular check ups with your general dentist are maintained throughout your orthodontic treatment. Before any braces are placed, the patient should have finished with the general dentist and have had a recent cleaning. Many times it is prudent for the patient to be seen in an alternating pattern by a dentist and a periodontist. This can be discussed by the patient and Dr. Hyduk, and the general dentist if necessary. Office Coverage Our offices have coverage during most regular business hours. In an extreme emergency, you may contact Dr. Hyduk. If you are unable to reach him, you may then try to contact your general dentist, or go to the emergency room of a nearby hospital. Dr. Hyduk is a medical staff member of Beaumont Hospital, Troy and Royal Oak, St. Joseph Mercy Oakland and Crittenton Hospital. Financial Arrangements Once your treatment fee and financial arrangements have been agreed upon and executed, you will receive a monthly statement from our office. We reserve the right to discontinue treatment if financial arrangements are not met. The responsible party and not the insurance company is responsible for the entire account. We accept personal checks, cash, money orders, VISA, MasterCard and Discover. There is a 5% discount offered for full payment up front (excluding credit card payments). A Truth in Lending form is used in orthodontic offices due to the nature of the financing. This form is for the protection of the person held responsible for the acount and is required by federal law. If you have insurance coverage for treatment, we will complete and submit the necessary forms. All insurance reimbursements will go directly to the subscriber. In the Event of a Transfer The fee shall reflect the treatment rendered and an appropriate adjustment of the balance will be made. All original records are to remain the the posession of our office. Duplicate records will be made with the necessary duplication fee charged to the responsible party. Upon payment, the records will be forwarded to the next treatment orthodontist upon request.
